2007 Confidential Patient Sign-In Sheets with Pegboard Binder
Privacy is now a very complex issue in the medical field. These confidential sign-in sheets are a simple solution to one aspect of ensuring your patient's privacy.
A patient signs in on a numbered slip. As the patient writes, the information is transferred via carbonless paper to a master log sheet. The patient's confidential information is protected behind the carbonless paper and is captured on the log for office use.
Once completed, the patient takes the sign-in slip. When the patient is called, the attendant reads the log and calls the patient by the slip's number.
Patients will no longer be able to see the previous patients' names and information when signing-in. Office personnel will refer to patients by number rather than by name.

These sign-in sheets come in packages containing 40 sheets. Each sheet has 25 slips. So one package will be sufficient for 1,000 patients.
You can order the packages with or without a pegboard binder. The binders are reusable so you can order one set with binder and then order refills.
