Payment can be made by any of the following options: Visa, MasterCard, American Express, Discover, personal or company checks, money orders, wire transfers or certified funds. Online orders can be paid for by credit cards.

Purchase orders will be accepted from companies and institutions who have established an account with us. If you wish to establish an account please click on the link below and print the pages. Fax or mail the completed forms to us at the address listed below.

Shipping Limits:

Aardvark Forms can ship to any residential or commercial address in the United States. At this time, shipments to other countries is limited. Please call our office at the number listed above for more information. Please see Shipping Methods page for more information.

Customer Guarantee:

All our products are warranted to be free from defect. Defective products will be replaced with another product of the same stock number. You may exchange a defective product within 30 days of receiving your order.

Return Policy:

We have instituted this return policy to give our customers explicit instructions on company procedures in the unlikely event that the need should arise to return a product for exchange or refund.

Please keep in mind that if a product is defective, we will gladly replace that product without delay, as long as you notify us within 15 days of receipt of the product. If you find that your product is defective, please call our office at 1-888-875-4798 between the hours of 9:00 am and 5:00 pm Eastern time, Monday through Friday, to receive a Return Authorization number (RA#). Aardvark Forms will immediately send a replacement for the defective product along with a return UPS label .

If a product is not defective, Aardvark Forms company policy provides for limited refunds within 15 days from the receipt of the product. These refunds depend on the product that you order. Each is addressed below:

· Unopened Books and Software: We will refund the purchase price minus a 25% restocking fee. You must call our office at 1-888-875-4798 between the hours of 9:00 am and 5:00 pm Eastern time, Monday through Friday, to receive a Return Authorization number BEFORE returning your product. Your product will not be accepted without a RA#. You will be responsible for shipping and insuring the product back to Aardvark Forms!

· Opened Books: Opened books can be returned for a credit on your account, minus a 25% restocking fee. You must call our office at call our office at 1-888-875-4798 between the hours of 9:00 am and 5:00 pm Eastern time, Monday through Friday, to receive a Return Authorization number BEFORE returning your product. Your product will not be accepted without a RA#. You will be responsible for shipping and insuring the product back to Aardvark Forms!

· Opened Software: No refunds are allowed on opened software. However, software can be upgraded in most cases. Please call our office for details.

· Training Materials, Special Offers, Forms, Envelopes or customized products: There are no refunds on any of these products, unopened or otherwise, unless defective.
